Aneurysms can form in any blood vessel in the body. An aneurysm is a dilation of an artery. Typically, an aneurysm is defined as 1.5 times the normal diameter of an artery. However, the size of an aneurysm requiring treatment varies based on anatomic site. One of the most common sites of aneurysmal disease is the popliteal artery. These aneurysms can usually be appreciated on physical exam. If a popliteal aneurysm is suspected this is usually confirmed with an ultrasound. Further imaging, usually with computed tomography, is required for surgical planning as well as to look for aortic aneurysmal disease as approximately 50% of patients with popliteal aneurysms have concurrent aortic aneurysms.
The fear with expanding aortic aneurysms is rupture and death. Alternatively, popliteal aneurysmal disease is associated with clotting and acute limb ischemia. All aneurysms have sluggish blood flow at the outer rim of the aneurysm causing a layer of thrombus to form. In popliteal aneurysms, the lumen becomes narrowed with thrombus and can occlude, causing an acute loss of flow to the lower leg and acute limb ischemia, which can also lead to limb loss requiring amputation. This is far less likely to occur in aortic aneurysms.
Several treatments are available for popliteal aneurysms. The aneurysms can be repaired in an open as well as an endovascular fashion. The type of repair is dictated by patient anatomy and more so by the patient’s comorbidities.
- Open Repair – An open repair requires an epidural at a minimum, but most commonly general anesthesia. Even among open repairs, surgeons can choose to ligate and bypass the aneurysm or completely resect and replace the artery. Resecting and replacing the aneurysm with an interposition graft requires the patient to be prone which can be challenging for anesthesia depending on the patient’s body habitus and pulmonary function.
- Endovascular Repair – Endovascular repair does not require general anesthesia and generally very well tolerated. Most patients require only local anesthesia with minimal sedation. Computed tomography (CT) is needed for surgical planning. Endovascular repairs require a covered stent to be placed across the popliteal joint and across the aneurysm itself. Patients can typically be discharged the following day.
